Patient Info
Your initial visit to our dental office is an important one. We take the time to do a thorough examination and find out what is most important to you. Dr. Patil, in coordination with your Dental Hygienist, will recommend the appropriate type of dental cleaning for you, based on the condition of your oral health.
Your initial comprehensive exam will include the following:
- Discussion regarding your dental concerns and expectations
- Review your dental and medical history
- TMJ (temporomandibular joint) Assessment- (jaw joint)
- Periodontal Assessment/diagnosis of your gum health
- Take any necessary dental records (X-rays and photographs)
- Oral Cancer Screening
- Orthodontic Assessment: an evaluation of your bite
- Evaluation of the condition of existing dental work
- Check for cavities/cavity risk assessment
- Initial Teeth Cleaning
To assist us in maximizing your appointment time, please complete your online patient registration, which includes a Medical History. List all medications that you are currently taking. If you are unable to complete your forms in advance, please plan to arrive 15 minutes before your appointment to complete them. For your convenience, please click the link and complete it.
X-Rays
If your previous dentist has taken recent x-rays, you may request that they forward them to our office. If you prefer not to have them forwarded, a new set of digital x-rays can be taken at your first visit.
Scheduling
Our office is open Monday, Tuesday, Thursday from 8:00 AM to 5:00 PM, Wednesday 8:00 AM -3:30 PM and Friday 7:00 AM to 1:30 PM. We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day. If circumstances arise that make it difficult to keep your appointment, please notify us at least 2 business days prior.
